Understanding Your Target Eye Pressure
Glaucoma treatment is not based on reaching one universal eye pressure. Instead, each patient has an individual target range based on the condition of the optic nerve and the risk of future vision loss. Understanding target eye pressure can help patients see why glaucoma treatment and regular monitoring remain important even when their vision appears unchanged.
What Is Target Eye Pressure?
Eye pressure is created by fluid that circulates within the eye. Normally, this fluid drains at approximately the same rate that it is produced. When drainage is not working effectively, pressure may rise and increase the risk of optic nerve damage.
A target eye pressure is the range an ophthalmologist believes will help reduce the risk of additional glaucoma damage. It is not the same for every patient.
The doctor may consider:
- The type and severity of glaucoma
- Existing optic nerve damage
- Peripheral vision test results
- Corneal thickness
- Previous eye pressure measurements
- The rate at which glaucoma is progressing
The target may also change over time based on examination findings and the patient’s response to treatment.
When Eye Drops Are Not Enough
Prescription eye drops can effectively lower pressure, but they must be used correctly and consistently. Some patients need more than one medication to reach their target.
Missed doses, side effects, difficulty administering drops, and complicated schedules can affect treatment. Even when medication is used properly, it may not always provide the amount of pressure reduction needed.
In these situations, an ophthalmologist may recommend laser treatment or surgery.
Reaching a Lower Pressure With SLT
Selective Laser Trabeculoplasty, commonly called SLT, is an office based laser procedure that improves the eye’s natural drainage system. Better fluid drainage may help lower eye pressure and reduce stress on the optic nerve.
Potential benefits of SLT include:
- No surgical incisions or stitches
- Minimal discomfort and recovery time
- Less reliance on prescription drops for some patients
- The possibility of repeating treatment when appropriate
SLT may be considered as an initial treatment for certain patients with open angle glaucoma or ocular hypertension. It may also be used alongside medication or when eye drops are not achieving the patient’s target pressure.
Results vary, and follow up testing is necessary to determine how effectively the treatment is working.
MIGS and Cataract Surgery
Minimally Invasive Glaucoma Surgery, or MIGS, includes several small incision procedures that may help improve eye pressure control. Depending on the procedure, MIGS may improve natural drainage, create another pathway for fluid, or reduce fluid production.
MIGS is often considered for eligible patients who are also undergoing cataract surgery. Potential benefits may include:
- Small surgical incisions
- Less disruption than traditional glaucoma surgery
- A relatively brief recovery
- Reduced dependence on glaucoma medication for some patients
However, MIGS may not provide enough pressure reduction for every patient. More advanced glaucoma may require traditional surgery or another treatment approach.
Monitoring Progress Over Time
Reaching the target eye pressure is only one part of glaucoma care. Your ophthalmologist will continue examining the optic nerve and testing peripheral vision to determine whether the disease remains stable.
